Provides a supportive online community for cancer patients and survivors. It offers personalized resources, expert guidance, and tools to manage treatment, track progress, and connect with others facing similar experiences.

A free online platform that helps cancer patients and their families stay connected during treatment and recovery. It allows users to share health updates, receive support from loved ones, and create a personal journal in a private and secure space.

Survey Instructions for Cancer Survivorship Care Portal

The survey is part of a Doctor of Nursing Practice (DNP) project on using a web-portal to share survivorship care information. Each set of surveys will take about 3 minutes to complete, and your answers will be kept confidential and will only be used for the DNP project.

  • Part 1: Complete Survey A and C before you interact with the web site.
    • In two weeks, you will receive a reminder to complete Part 2 below (Surveys B and C). 
  • Part 2: Complete Surveys B and C at least two weeks after you have had the opportunity to engage with the website.
