Implementing a survivorship care program requires a thorough needs assessment and the establishment of models for long-term follow-up care.

Clinical guidelines for survivorship care vary by cancer type and provide specific recommendations for follow-up care, monitoring, and management of long-term effects. These guidelines help ensure that survivors receive tailored care based on their individual needs and treatment history.

Follow-Up Care Recommendations

Follow-up care is essential for monitoring for cancer recurrence, managing side effects, and maintaining overall health. Recommendations typically include regular physical exams, imaging tests, and screenings based on the type and stage of cancer. A personalized follow-up care plan helps survivors feel more in control and supported in their post-treatment journey.

Survey Instructions for Cancer Survivorship Care Portal

The survey is part of a Doctor of Nursing Practice (DNP) project on using a web-portal to share survivorship care information. Each set of surveys will take about 3 minutes to complete, and your answers will be kept confidential and will only be used for the DNP project.

  • Part 1: You will first complete Survey A and C before you access the website content.
    • You will receive a reminder e-mail to complete Survey B and C, a few weeks Part 1.
  • Part 2: You may complete Survey B and C at any time after you access the website content.
