What is a Survivorship Care Plan?

A survivorship care plan is a document that tracks your cancer treatment, follow-up care, and ways to stay healthy. It helps you and your doctors manage your health after treatment. The plan includes your treatment history, possible side effects, needed check-ups, and health tips.

How Do I Get a Survivorship Care Plan?

  1. If the facility where you received treatment has a survivorship program to assist you after treatment ends, they will have a process set up to create a Survivorship Care Plan.
  2. You may use an online tool to create a free, personalized plan.  Use the OncoLife™ Tool: Visit the OncoLife™ Survivorship Care Plan to create your plan. This free and simple program helps cancer survivors understand health risks related to their past cancer treatments.

 

How Do I Create and Use a Survivorship Care Plan?

  • Create your plan
    • Go to OncoLife™ Survivorship Care Plan
    • Click “Develop My Cancer Survivorship Care Plan”
    • Select the “Patient Version”
    • Answer questions about your cancer type, treatments, health history
    • Print or download your plan once complete
  • Review your plan
    • Bring your plan to your next doctor’s visit
    • Ask questions about follow-up tests, side effects, lifestyle tips
    • Make sure your doctor agrees with the plan and adjust if needed
  • Follow your plan
    • Keep up with the recommended check-ups and screenings
    • Monitor and report any new symptoms
    • Follow tips for staying healthy
  • Share your plan
    • Give a copy of your plan to your primary doctor and other members of your healthcare team
    • Use your plan to track your medical history and future care
  • Update your plan
    •  Review your survivorship care plan every year and update it with new treatments or health concerns

Survey Instructions for Cancer Survivorship Care Portal

The survey is part of a Doctor of Nursing Practice (DNP) project on using a web-portal to share survivorship care information. Each set of surveys will take about 3 minutes to complete, and your answers will be kept confidential and will only be used for the DNP project.

  • Part 1: You will first complete Survey A and C before you access the website content.
    • You will receive a reminder e-mail to complete Survey B and C, a few weeks Part 1.
  • Part 2: You may complete Survey B and C at any time after you access the website content.
