How Do I Get a Survivorship Care Plan?

How Do I Create and Use a Survivorship Care Plan?
- Create your plan
- Go to OncoLife™ Survivorship Care Plan
- Click “Develop My Cancer Survivorship Care Plan”
- Select the “Patient Version”
- Answer questions about your cancer type, treatments, health history
- Print or download your plan once complete
- Review your plan
- Bring your plan to your next doctor’s visit
- Ask questions about follow-up tests, side effects, lifestyle tips
- Make sure your doctor agrees with the plan and adjust if needed
- Follow your plan
- Keep up with the recommended check-ups and screenings
- Monitor and report any new symptoms
